I certify that I have reviewed all of the information provided on the application and verify that it is true and accurate. I am aware that false misrepresentation of my age on this application subjects me to criminal prosecution. Accordingly, I must be twenty-one (21) years of age or older to apply for credit with HRHCAC. I understand that persons under the age of twenty-one (21) are not permitted to gamble. I hereby authorize Boardwalk 1000, LLC (dba Hard Rock Hotel & Casino Atlantic City) ('HRHCAC') to obtain a consumer credit report and to conduct such other investigations as it deems necessary, and to furnish information concerning my credit record to other credit-reporting agencies and others who may properly receive this information.
